Did anyone disagree after the meeting who said nothing during it?
Was a number or plan presented with more confidence than at least one person in the room actually felt?
Was a decision nodded through that some people privately doubted?
Did anyone start to say something difficult, read the room, and soften it mid-sentence?
Did a known issue stay unraised because nobody had a polished solution for it yet?
Was a decision everyone already understood sent for more analysis that will not change it?
Did something everyone knows needs to happen wait for a formal approval that is really a formality?
Was input invited on something that had clearly already been decided?
Did whoever raised the hardest point end up carrying it alone, with nobody seconding them?
Did the meeting end in agreement, while the real conversation started afterwards, in twos and threes?
